This page explains, for reviewers and patients alike, how Pecan Dental and Implant Solutions LLC d/b/a Smile Makeover obtains consent before sending appointment- and service-related text messages, and how that differs from optional marketing text messages. There are three distinct, documented opt-in paths, described below in the order a visitor is most likely to encounter them.
Method 1 — Official website contact form (publicly verifiable, same domain)
Live, public, no login required: https://www.smilemakeoverdallas.com/#contact
The contact form at the bottom of our homepage includes two independent SMS checkboxes:
- Service / patient-care SMS — unchecked by default, optional. Covers appointment scheduling, confirmations, reminders, rescheduling/cancellations, consultation follow-up, requested forms/photos, treatment or case status updates, insurance/billing administrative messages, secure payment links, post-visit follow-up, and recall messages.
- Marketing / promotional SMS — a completely separate checkbox, also unchecked by default and optional. Never bundled with the service checkbox.
Both checkboxes' visible text includes: message frequency varies; message and data rates may apply; reply STOP to opt out or HELP for help; and links to our SMS Terms and Privacy Policy. The "not a condition of" wording is written independently for each, and is not identical between them:
- Service / patient-care SMS reads: "Consent is not a condition of receiving dental care or submitting this form."
- Marketing / promotional SMS reads: "Consent is not a condition of receiving dental care, booking an appointment, or purchasing any service."
Key behaviors, enforced both in the browser and again on our server (so they can't be bypassed):
- The form can be submitted with neither box checked. SMS consent is never required to send a general inquiry.
- The "Preferred Method of Communication" field (Phone / Text / Email) is a separate, ordinary contact preference. Selecting "Text" alone is never treated as consent — if a visitor selects Text without checking the service-SMS box, the form shows an inline message asking them to either opt in above or choose Phone or Email instead, and will not submit until one of those is true.
- Marketing consent is never required, and is never sufficient by itself to enable service messaging.
Each submission is recorded with the exact checkbox states, a UTC timestamp for each consent given, the specific consent text version shown, the submitter's IP address and browser user-agent, and a fixedconsent_source value identifying this form — so every opt-in captured here has a verifiable evidence trail.
Method 2 — Flourish booking flow
Note: our homepage's primary appointment button links to Zocdoc, a third-party scheduling directory, for general booking. The flow below is Smile Makeover's separate, SMS-enabled booking system, hosted by Flourish.
Live booking page: https://fluorish.ai/booking/osama-siddiqui-DQ4DSk
Live compliance evidence page (documents the exact consent wording and checkbox behavior, read live from our configuration): https://fluorish.ai/sms-consent/osama-siddiqui-DQ4DSk
A reviewer can open either link directly. In the booking flow:
- After selecting an appointment and choosing "Text me a code" at the phone-verification step, two independent, unchecked-by-default consent checkboxes are shown together.
- Checkbox 1 (patient-care / service SMS) covers scheduling, confirmations, reminders, rescheduling/cancellations, consultation follow-up, requested forms/photos, treatment or case status updates, insurance/billing administrative messages, secure payment links, and post-visit/recall messages.
- Checkbox 2 (marketing/promotional SMS) is separate and optional, and is never required alongside checkbox 1.
- The send-code action is gated only by the patient-care checkbox — marketing consent alone never enables SMS sending, and is never required to receive care, book an appointment, or purchase any service.
- A patient who wants no SMS at all can choose "Email me a code" instead; that path requires zero SMS consent and completes booking normally.
- Both checkboxes link directly to our SMS Terms and Privacy Policy.
Because this flow reflects live, real patient scheduling, we link to it directly above rather than reproducing screenshots of a live patient-facing form.
Method 3 — Verbal / AI-assisted one-time SMS path
When a patient calls our office (including our AI-assisted phone line, built on the Fluorish AI platform) and asks about booking, our AI receptionist may ask the following exact question, read live from Smile Makeover's own Fluorish AI configuration, before any link is texted to them — see the live evidence page, Section 3 for the current wording, reproducible at that link at any time:
Only an affirmative response authorizes that one-time text message containing the requested link. This authorization covers only that one requested message — it does not itself establish recurring marketing consent, and does not enroll the caller in recurring patient-care SMS; that requires the applicable separate opt-in flow described above. This is not a keyword opt-in, and we are not using keyword-based opt-in (such as texting a keyword to a shortcode) for this campaign.
What this does not include
Other web properties associated with this practice that are not one of the three methods documented above (for example, independently operated marketing landing pages) are not part of this SMS program and are not relied upon as opt-in evidence here, regardless of any SMS-related language they may separately display.
